Social Worker/ Regional Program Manager Position
Summary:
HFM Social Worker/Regional Program Manager includes the facilitation of the financial assistance program and oversight of the Delta Dental insurance program, as well as providing resources for the hemophilia treatment centers (HTC) and bleeding disorder community members.
In addition, the Social Worker/Regional Program Manager works via the Regional Core Center (RCC) in support of the federal Great Lakes hemophilia treatment centers across Michigan, Ohio and Indiana This position provides support for foundation events and programs.
Other duties may be assigned to meet organizational needs.
Essential Duties and Job
Responsibilities:
Social Worker Responsibilities Provide support for health education in conjunction with program services staff via various platforms (social media/website/programs/events).
Provide state and local resources in coordination with Michigan HTC social workers to support patients via HFM's financial assistance and resource efforts.
Work with operations colleagues to provide oversight for the Delta Dental program.
Support Michigan hemophilia treatment center initiatives in partnership with treatment center staff.
Provide support for The Artery newsletter preparing content and securing HTC staff as authors.
Support program services events and programs as necessary.
Offer leadership and collaboration regarding HFM health diversity, equity, and inclusion initiatives.
Manage the annual HFM scholarship program.
Manage various Michigan support groups in partnership with the group facilitators.
Assist in supporting social work engagement with HFM camping programs.
Facilitate quarterly Michigan HTC Social Worker drop-in meetings or educational webinars and include regional HTC social worker meetings twice a year.
Regional Program Manager Responsibilities Plan and execute state and regional HTC staff meetings in partnership with RCC staff and HTCs.
Coordinate regional social work accreditations as necessary for HTC social workers.
Facilitate quarterly Michigan HTC social worker drop-in meetings or educational webinars.
Work in partnership with RCC staff and committee to plan and execute the annual Regional Meeting.
Provide continuing education opportunities for regional HTC staff as appropriate.
Engage regional HTC staff in HFM programs and events as participants and speakers.
Perform site visits to regional HTC social workers to enhance working relationships.
Facilitate regional newsletter.
Familiarity and support of MCHB/HRSA funding requirements and reporting.
Other duties as assigned Qualifications/Requirements:
Excellent written and oral communication skills.
Excellent interpersonal relationship skills to work with a diverse patient population and hemophilia treatment center staff.
Familiarity with social media in health promotion.
Creative and energetic in development of new activities.
Must be detail and results oriented; work well with deadlines.
Driver's license required.
Education/Training/
Experience:
Master's degree in social work.
Clinical social work license or working toward licensing.
Community experience preferably with chronic disease population.
Proficient in Microsoft Office.
Experience with event planning.
Experience with developing and facilitating webinars.
Physical Requirements:
Ability to safely and successfully perform the essential job functions consistent with the ADA and other federal, state and local standards, including meeting qualitative and/or quantitative productivity standards.
Ability to maintain regular, punctual attendance consistent with the ADA and other federal, state and local standards.
